The Auxiliary to Windham Community Memorial
Hospital is holding its 12th annual "Autumn
Gala" on Saturday, November 17, 2007 at
the Marriott Ballroom at Adrian's Landing
in Hartford. An integral part of the festivities
is the recognition of both the Physician
and Caregiver of the Year.
We hope every member of the community will
consider nominating their physician or
other caregiver for one of these prestigious
awards. You will find the nomination form
on the reverse side of this letter.
Criteria for consideration
Currently affiliated with Windham Hospital
Active in the affairs of the Hospital
Participates in community activities
Contributes to the improvement of health
in the community
Exemplifies the best in the Physician/Caregiver-patient
relationship
Completed nominations must be submitted
by August 17, 2007 to:
