WINDHAM ONE OF THREE HOSPITALS IN STATE TO BE AWARDED GRANT
The Windham Hospital Foundation was awarded a grant of $4,960 from the Connecticut Office of Rural Health to improve outcomes for patients served by the hospital. The grant will assist Windham Hospital’s efforts to reduce the number of readmissions to the hospital from patients with heart failure.
Barbara Berger, Director of the Connecticut Office of Rural Health, stated, "This program is one of the very few that focuses on the issues of health care for our rural residents."
The quality initiative will provide two targeted education programs for staff, community providers, local nursing facilities and home care agencies to help increase compliance with discharge instructions. As a result, there will be standard messages across the healthcare continuum that will reduce the rate of readmissions.
“It really is an honor for our hospital to be chosen to receive this grant money,” said Dick Brvenik, President and CEO, Windham Hospital. “Reducing readmission rates is a goal all hospitals across Connecticut share and certainly Windham Hospital is pursuing measures daily to realize this goal.”
The CT Office of Rural Health oversees the Rural Hospital Quality Improvement Grant Program, which focuses on enhancing health care for those persons living in the rural areas of Connecticut. The resources for this program originated from the Federal State Office of Rural Health Grant Program, funded through the Department of Health Resources and Services Administration (HRSA).
