Position Title: Case Manager

Department:Case Management
Position Description: Current state of CT RN license, BSN or BS in related field is required. Ability to utilize Microsoft Office strongly preferred, experience with MIDAS software a plus. Also require experience in any of the following areas: discharge planning, utilization management, case management or home care. Certification as Case Manager or Utilization Management preferred. Individual will be responsible for reviewing all hospital admissions and continued stays for appropriateness. Assess readiness of and facilitate the transition of patients to alternative levels of care. Ensure that patient receive the appropriate services at the appropriate time to promote quality outcomes at reasonable cost.

Shift Time Schedule:7:45 a.m. - 4:15 p.m.
Days:Sunday - Saturday
